While the player's head is submerged in Lava, throwing a Blank Realmstone out of their inventory will convert it into a Nether Realmstone. The Realmstone reappears in the player's inventory when this is done successfully.

Using a Nether Realmstone on a Carved Rune of Power at a Portal will create a portal to the Nether within the frame.
