Obtaining

Graw Altars are unbreakable by all means. They cannot be obtained in Survival mode normally.

Natural Generation

One Graw Altar spawns at the top of each Graw Pillar.

Usage

Graw Altars are used to spawn the boss Graw. This can be done by pressing use on the block with a Guardian's Eye in the player's inventory.
